Add the option to add custom fields or templates to the request form. This would allow us to add a version number field to our bug reports board for instance.

Yes this would be very important for us.
In our case it would be to able specify the version number of the bug or feature being requested.
Ideally we could populate the field with some choices like in a combobox or something.
Would be nice also if this could be allow to be passed from an application automatically.

This would be incredibly helpful, the best real world example i can find is attached from debank. Each category is linked to a specific templates with custom required or non-required fields.
